Information Gathering in Implementation Project

Hello fellow Admins/Consultants,

I've been wondering for a while, and looking for, an Excel document or something similar, to do information gathering in a client's environment, for a vSphere implementation project. I haven't been able to find one, and most of the ones I have were improvised by me kind of on the go. If anyone has one that wants to share, or point me to a good template, I would be grateful.

You might find this is useful to some degree but it is a bit out of date. It's more a design document than for implementation gathering.
