After we remove appstacks from a user we still have a cache looking icon that stays on the desktop or taskbar. How can we make sure all these are removed with the AppStack removal.

Are you using some kind of profile or persona management tool? If so, is it possible that software is grabbing the contents of the folders and trying to place them back?
I am using UEM so that is a good point, but it happens immediately when the appstack is removed. It leaves the icon place marker, not the real icon. Hope that makes since.
That is often referred to as a broken icon. Using ProfileUnity I have run into it. Often times it happens when a shortcut is set to a executable that does not exist anymore, or it is placing it over a icon that already existed. Check the UEM share location that has the start menu icons. If it is, you will want to create a exception for the App Volumes shortcuts.
Also, you could test on a system not running UEM, see if you have the same issue.
if the application places an icon into for example c:\users\public\desktop or systray etc then when that app is removed it can leave a broken icon. This is known issue described in the release notes. Generally speaking it can be fixed by modifying how the provisioning is done however things like the quick launch bar are difficult to account for without using additional tools like UEM to manage the icons.
