Usually the responsibility of a storage team is to provide disk space to the hosts and make sure they can meet the space and I/O requirements. Creating a VM is usually done by the VMware administration.

The basic informations the storage team needs are WWN's (in case of FC storage) or IP addresses and iqn's (in case of iSCSI) to setup the hosts. In addition to this the need top be aware that it is a VMware environment. Depending on the storage systems, the hosts need to be defined with the appropriate OS which uses the LUNs. Once the hosts are configures, all you need to do is to tell the storage team what size of LUN you need, to which hosts the LUNs should be presented and probably the expected I/O for the VM's running on this LUN, so they can make the decision for e.g. RAID levels, tiering...

Once the storage has been setup and all your hosts can see the shared storage, you may be able to move (VMotion, sVMotion or cold migrate) your VM's as you like. However, you should probably talk with the storage team in case the load on a datastores changes significantly. If the storage cannot meet your needs, you (your VMs) will be affected as well.
